Irregular Periods

A normal cycle runs between 21 and 35 days, counted from the first day of one period to the first day of the next. It doesn't have to be exactly 28 days, and a variation of a few days from month to month is normal. Periods are called irregular when the gap keeps changing a lot, or when cycles are regularly shorter than 21 or longer than 35 days.

Common causes

●       PCOS/PMOS, one of the most common reasons, often with acne, excess hair growth, or weight gain

●       Thyroid problems, both underactive and overactive

●       Stress, sudden weight loss or gain, and over-exercising

●       High prolactin levels

●       Hormonal contraceptives, including when you start or stop them

●       Perimenopause, usually after the late 30s or 40s

●       Teenage years and after delivery, when cycles take time to settle.

When to see a gynaecologist

●       Cycles shorter than 21 days or longer than 35 days, repeatedly

●       No period for 3 months or more, to rule out pregnancy or other causes (e.g. an ovarian cyst)

●       Very heavy or very painful periods along with irregularity

●       Irregular periods with acne, hair growth, or weight changes

What the evaluation involves

Usually a short history, an examination, a pelvic ultrasound, and a few hormone and thyroid tests if needed. Most causes are treatable, and early treatment protects your fertility and long-term health.
